Comprehensive cover (Vollkasko) is usually worth it for new, financed, leased and expensive cars, roughly in their first three to five years. It also pays for accidents you cause and for vandalism. Once the car is worth only a few thousand euros, or the cover costs more than a tenth of that value each year, partial cover (Teilkasko) is often enough.

Over three years the cover costs about a sixth of the car's value. If you have EUR 5,000 in savings, switching to partial cover is reasonable. Worked example with guide values.
With leasing or a car loan, the lender almost always requires it. Beyond that: if a total loss you caused yourself would hit you hard financially, you should have it.
For new drivers it is expensive, but that is exactly when it protects you from the cost of a driving mistake on your own car.
German comprehensive cover is similar to collision plus comprehensive in the US, but it is priced through your no-claims class. If your driving history from home is not recognised, you start in a low class and the own-damage part is expensive at first.
In your first years in Germany, driving on unfamiliar roads with a car you may still be paying off, the cover protects you most. Many newcomers also buy a used car. Then the question is the car's value, not how long you have lived here.
Compare the extra cost over partial cover with the car's current value. If the extra cost over two or three years eats up a large part of the value, comprehensive cover hardly pays off.
Include the step back in your no-claims class: after a reported claim you pay more for several years.

If you could replace the car yourself after a total loss and it is worth little, you usually pay in more than you will ever get back.
To be honest: many people keep paying out of habit. A yearly look at the car's value is worth it.
