It is worth it if you could not pay a vet bill of EUR 2,000 to 4,000 for an operation out of your own pocket. Surgery cover (OP-Schutz) often costs EUR 5 to 25 a month, full cover (Vollschutz) EUR 14 to 75. If you keep solid savings just for your dog, insuring only operations or nothing at all can be enough.

A single operation costs as much as ten to sixteen years of surgery cover.
German vets charge according to an official fee scale. Since November 2022 a general examination costs EUR 23.62 at the single rate instead of EUR 13.47 before.
Vets may charge up to three times the single rate, and more in emergency service at night or at weekends. An operation with a stay at an animal clinic can quickly reach several thousand euros.
Surgery cover pays only for operations, including the treatment before and after. It protects you against the big bills for a low premium.
Full cover also pays for treatment without an operation, such as medication or allergy therapy. It is more useful for breeds with known hereditary conditions.

In the UK you may know lifetime and time-limited pet policies. In Germany the terms to compare are different: the share of the bill that is refunded, the annual limit (Jahreshöchstgrenze) and up to which multiple of the fee scale the insurer pays.
Dog health insurance is voluntary. Separate from it is dog liability insurance (Hundehaftpflicht), which pays for damage your dog causes to other people and is mandatory in several federal states such as Berlin, Hamburg and Lower Saxony. Illnesses your dog already had before you signed, including in your home country, count as pre-existing and are usually excluded. Policy documents are mostly in German.
If you keep a fixed reserve of a few thousand euros just for your dog, you can carry the risk yourself. Over a dog's life, full cover often costs several thousand euros in premiums.
For older dogs a new policy becomes expensive or is no longer offered at all, and existing illnesses are excluded.
