Total disability insurance in Germany: the fallback when occupational cover is out of reach
Erwerbsunfähigkeitsversicherung, or EU for short, pays a monthly benefit if illness or injury leaves you unable to work three hours a day in any job at all. It does not look at your own occupation, which makes it cheaper than occupational disability insurance (BU) and much harder to claim on. We explain plainly when it is worth it and when it is not.
Is total disability insurance as good as occupational disability cover?
No. A BU policy pays when you can no longer do your own job to at least 50 percent. An EU policy pays only when you cannot manage three hours a day in any work on the general labour market, roughly the same test the German state uses for a full reduced earning capacity pension. It makes sense if a BU application was declined or the premium for your occupation is too high. Stiftung Warentest, the German consumer testing body, describes it as considerably cheaper for some occupations.
What three hours on the general labour market means in practice
The yardstick is any job that exists, from receptionist to call centre agent, not the one you trained for. A roofer with a ruined knee who could still sit at a desk for four hours a day is not totally disabled, even if he never climbs a roof again. A BU policy would pay him; an EU policy would not.
Policy wordings are not identical. Some pay at exactly three hours, others only below three. Severe illnesses such as advanced cancer, a stroke with lasting damage or a major depression often reach the threshold. A bad back, an allergy or losing the use of one hand usually do not.
Why the German state will often not step in for newcomers
The statutory pension insurance uses the same test: under three hours a day gives a full reduced earning capacity pension, three to under six hours gives half. To qualify you normally need five years of insurance, three of them with compulsory contributions in the last five years. If you have only worked in Germany for a short time, check whether insurance periods from another EU country count towards this before relying on it.
Even with a full record, the amounts are modest: according to the annual report of the Deutsche Rentenversicherung, newly granted reduced earning capacity pensions averaged around 1,067 euros a month in 2025. A private EU policy pays on top, and it does not require any contribution years in the state system, which matters for the self employed and for recent arrivals.
EU, BU or basic abilities cover: an honest ranking
Occupational disability insurance (Berufsunfähigkeitsversicherung) is the strongest cover because it measures against your own job. Basic abilities insurance (Grundfähigkeitsversicherung) pays when a defined ability such as walking, seeing or using your hands is lost, but often leaves mental illness out. Total disability insurance does not care about the cause at all, only about whether you can still work.
So try BU first, with an anonymous risk enquiry if you have a medical history. If it is declined or unaffordable, compare basic abilities and total disability cover side by side. Manual workers are often better served by basic abilities cover; people mainly worried about mental illness are often better served by EU cover.
What is covered
Monthly benefit when you cannot work three hours a day in any job
Any cause: illness, accident or decline of strength, including mental illness
Premium waiver while the benefit is paid
Backdated payment from the start of the disability, depending on the policy
Separate benefit if you need long term care, depending on the policy
Guaranteed increase of the benefit once it is paid, if agreed
What is not covered
Losing only your own occupation while other work for three hours a day is still possible
Partial limitation, for example four or five hours of remaining capacity
Pre-existing conditions not disclosed in the application, whether treated in Germany or abroad
Exclusions agreed when the insurer accepted you
Self inflicted injuries
Who needs it
People whose BU application was declined or offered only with heavy exclusions
Occupations in high BU risk classes where the premium is not affordable
Self employed people and recent arrivals without a claim to the state pension
Anyone who wants a second layer of cover for the worst case next to a small BU
People mainly worried about mental illness, which basic abilities cover often excludes
Who does not need it
If you can get an affordable BU policy, because it pays in far more situations
If your savings would carry you to retirement without earned income
Wheel of misfortune
Spin the wheel of bad luck
Six things that really happen. The wheel picks yours and shows who pays.
Cost:
Budget policy Strong policy
See all six cases
Major depression, unable to work, with 1,500 € a month: 18,000 € a year. Budget policy: pays in part, Strong policy: pays. If it is clear you cannot work three hours a day, the policy pays. With a permanent prognosis, proving it takes longer; with a six month prognosis the money arrives sooner.
Roofer with a ruined knee, nothing, as long as desk work is possible. Budget policy: does not pay, Strong policy: does not pay. He can never go back on a roof but could still work six hours in an office. That is occupational disability, not total disability. Only BU or basic abilities cover helps here.
Stroke with paralysis, monthly benefit until the end age, six figures over a long term. Budget policy: pays, Strong policy: pays. Lasting paralysis and speech problems leave hardly any capacity for work. This is exactly the case EU cover is built for.
Exactly three hours left, full benefit or nothing. Budget policy: does not pay, Strong policy: pays. Policies that pay only below three hours decline. Policies that accept three hours or less pay.
Care grade after an accident, monthly benefit plus care costs. Budget policy: pays in part, Strong policy: pays. Usually total disability applies anyway. With a separate care benefit, the policy pays without any dispute about working hours.
Cancer at 62, cover ends at 63, four years of benefit, at 1,200 € about 57,600 €. Budget policy: pays in part, Strong policy: pays. If the benefit term ends early, payments stop at 63. Cover until 67 bridges the gap to the state retirement pension.
What it costs
Because the insurer only pays when you can hardly work at all, the premium is lower than for a BU policy with the same benefit. The gap is widest for manual occupations, where BU is expensive and EU weighs the occupation less heavily.
What drives the premium
Age at entry and state of health
Monthly benefit
Term of the policy and of the benefit, usually until 67
Occupation, often in broader classes than for BU
Smoker status
Indexation before and after a claim
We do not quote a premium range, because age, health and benefit level drive the price. Your premium is fixed in the quote.
The policy levels on the market
01
Standalone EU policy
A separate contract with a monthly benefit and premium waiver. The usual route when BU is not an option.
02
EU with care benefit and indexation
Adds a benefit if you need long term care, guaranteed increases during a claim and options to raise cover later.
03
EU as a rider
Attached to a term life or pension policy. Cancelling the main contract usually ends the rider too.
How to recognise a good policy
Criterion
Minimum standard
Strong policy
Why it matters
Threshold in the wording
less than three hours a day
three hours a day or less
That one hour decides in borderline cases whether anything is paid at all.
Prognosis period
expected to be permanent
six months, with backdated payment
A permanent prognosis is harder to prove and delays the first payment.
Benefit term
until 60 or 63
until 67
A gap before the state pension cannot be closed later.
Option to increase cover
on marriage and birth of a child
many events, including a pay rise or becoming self employed
Cover should grow with income without new health questions.
Care benefit
none
benefit on a recognised care grade, regardless of working capacity
Someone who needs care should not have to argue about working hours.
Premium deferral
deferral in payment difficulties
interest free deferral, also during parental leave and unemployment
A cancelled policy is often impossible to get back with new health questions.
Insurers on the German market
Far fewer German life insurers offer total disability cover than occupational disability cover. The companies below appear in rating agency comparisons for this line. This is a market overview, not a statement about which insurers we work with.
Insurer
Background
Continentale
part of the Continentale group, agents and brokers
Dialog
specialist for biometric risks, sold through brokers
Europa
direct insurer based in Cologne
Hannoversche
direct insurer, part of the VHV group
HDI
part of the Talanx group, agents and brokers
WWK
mutual insurer from Munich
Volkswohl Bund
mutual insurer from Dortmund, sold through brokers
Covers only the risk of not being able to work at all. Independent of any savings plan, so it stays flexible.
Occupational disability insurance (BU)
Measures against your own job and therefore pays much earlier. The better choice whenever you can get it at a price you can carry.
Basic abilities insurance
Pays when defined abilities such as walking, seeing or gripping are lost. Good for manual jobs, weak on mental illness.
Critical illness insurance
A lump sum on diagnoses such as cancer, heart attack or stroke. Complements a monthly benefit, does not replace it.
Accident insurance with annuity
Pays only after accidents, not for illness. Not a substitute for income protection.
Myth or truth
True or false?
Five things people say about this insurance. Guess first, then see the answer.
Total disability cover pays when I can no longer do my own job.
This is false.
False. That is what BU does. EU cover only pays when you cannot do any work for three hours a day.
There are no health questions for total disability cover.
This is false.
False. The insurer still asks about your medical history. The assessment is often a little milder than for BU, and some people declined for BU are accepted here.
The German state uses the same three hour threshold.
This is true.
True. Under three hours a day gives a full reduced earning capacity pension. State and private benefits are triggered in roughly the same situation, but each assesses separately.
Total disability cover is cheaper than BU for the same benefit.
This is true.
True. Stiftung Warentest calls it considerably cheaper for some occupations. The price for that is a much higher hurdle when you claim.
Mental illness is excluded from total disability cover.
This is false.
False. The cause does not matter. If you cannot work three hours a day because of a severe mental illness, the policy pays. That is its advantage over many basic abilities policies.
Common mistakes
Taking EU cover without first seriously trying for BU
Assuming it pays as soon as you cannot do your own job
Leaving out treatment abroad when answering the health questions
Letting the benefit end before 67
Assuming the German state pension will fill the rest without checking the five year waiting period
Frequently asked questions
Questions and answers
Frequently asked
When am I totally disabled under the policy?
When illness, accident or decline of strength means you are expected to be unable,
permanently or for at least six months with good policies,
to do any work on the general labour market for three hours a day.
Your previous job and training hardly matter.
Does it pay for mental illness?
Yes, the cause does not matter.
What counts is whether you can still work three hours a day.
A severe depression can reach that threshold;
a milder one with four or five hours of remaining capacity cannot.
Do I have to answer health questions?
Yes.
They are often a little less strict than for BU, but they do not disappear.
Include treatment you received in your home country,
because false answers can later allow the insurer to withdraw from the contract.
Can I receive the state pension and the private benefit at the same time?
Yes.
The private benefit is not offset against the state pension.
The insurer assesses your case independently and is not bound by the decision of the Deutsche
Rentenversicherung unless the policy says so.
Who is EU cover right for?
For people who cannot get an affordable BU policy, and for the self employed without a state claim.
Stiftung Warentest considers the much cheaper total disability cover sometimes even more sensible
for the self employed.
What happens if I move away from Germany?
That depends on the terms.
Check the territorial scope before signing,
and expect that medical evidence from doctors abroad may need translating when you claim.
How does it differ from basic abilities insurance?
Basic abilities insurance pays when a specific ability is lost, even if you could still work.
EU cover pays when you cannot work at all, whatever ability is missing.
Manual workers are often better off with basic abilities cover.
