All about this insurance: Accident insurance
Do I need private accident insurance if I already have health insurance in Germany?

In short
Worth having, yes. The most important cover, no. Your child is insured by the statutory scheme in day care, school and on the way there, but not at home or in their free time. A private policy costs about EUR 50 to 100 a year. It only pays after accidents, and around 90 percent of severe disabilities in children are caused by illness. Only child disability cover handles that.
Guide figures from published tariff examples for Germany, plus the order of magnitude of a payout.
| Example | Typical range |
|---|---|
| Child accident insurance, base sum EUR 100,000, progression 225 to 350 percent | about EUR 50 to 100 a year, so EUR 3 to 8 a month |
| Child accident insurance with a high base sum at a low cost insurer | often EUR 40 to 80 a year |
| Difference between a cheap and an expensive insurer for the same cover | a factor of 2 to 3 in the premium |
| Child disability insurance, lump sum of EUR 100,000 only | from about EUR 107 a year |
| Child disability insurance with a lifelong pension and a lump sum | about EUR 275 a year, about EUR 300 with a pension of EUR 1,000 a month |
| Payout at a disability degree of 25 percent, base sum EUR 150,000 | about EUR 37,500, progression does not bite yet |
| Payout at 50 percent, base sum EUR 150,000, progression 350 percent | around EUR 150,000, a little more or less depending on the table |
| Payout at 100 percent, base sum EUR 150,000, progression 350 percent | EUR 525,000 |
Guide values as of September 2026. The premiums come from publicly quoted tariff examples, the child disability figures from a published comparison. The payouts at 25 and at 50 percent are guide values from quoting practice, because every progression table runs differently; only the figure at 100 percent follows directly from the progression. Your premium depends on the base sum, the cover and the insurer; we obtain the exact quote for you.
Worked example
What a recognised disability degree of 50 percent would pay, and what that sum is worth spread over a lifetime.
| Base sum (Grundsumme) | EUR 150,000 |
| Progression | 350 percent, capped at EUR 525,000 |
| Recognised degree of disability | 50 percent |
| Benefit | around EUR 150,000 (guide value from quoting practice) |
| Premium | about EUR 50 to 100 a year |
| Counter calculation over 80 years | EUR 150,000 divided by 960 months is about EUR 156 a month |
The sum sounds large and is not: spread over a whole life it leaves a top up of about EUR 156 a month. For a child the high base sum therefore matters more than any add on. And if the cause had been an illness, this policy would pay nothing at all.
Children in Germany are covered by statutory accident insurance (gesetzliche Unfallversicherung) in day care, at school and in after school care, on the way there and back, and on school trips and class journeys. Parents pay nothing for it. In 2024 more than 1.1 million such accidents were reported nationwide.
That cover stops at the garden gate. At home, in free time and in a sports club outside school there is no statutory disability benefit, and that is where most accidents happen: 43.8 percent occur in the private environment, 24.2 percent in care and school settings and 17.4 percent on playgrounds or sports grounds. A private policy closes that gap and applies around the clock, including abroad.
If you want to protect your child against a lasting severe impairment, accident insurance covers the smaller part of the risk. Figures from the federal statistics office show that around 90 percent of severe disabilities are caused by illness, about 3 percent are congenital or arise in the first year of life, and only about 1 percent come from an accident or an occupational disease. Around 200,000 children and young people under 18 in Germany are registered as severely disabled.
The statutory side shows the same thing: out of more than 1.1 million school and day care accidents in 2024, only about 578 new accident pensions were granted. Anyone who really wants to insure against lasting impairment should look at child disability insurance (Kinderinvaliditätsversicherung). It does not ask what caused the impairment, only how severe it is, and the usual trigger is a recognised degree of disability of 50 percent set by the local welfare office (Versorgungsamt). The price for that: it costs several times as much and asks health questions as detailed as income protection does.
The table shows which risk each route carries. The logic is simple: accident insurance is cheap because it covers little, and child disability insurance is expensive because it covers a lot.
Congenital impairments and illnesses already known when you apply are normally excluded from both private policies. That is why the contract has to be signed early, while the child is healthy, and not after the first diagnosis.
| Risk | Statutory accident insurance | Private accident insurance | Child disability insurance |
|---|---|---|---|
| Accident in day care, school or after school care | pays | pays | pays from the agreed degree |
| Accident on the way there | pays | pays | pays from the agreed degree |
| Accident at home or in free time | does not pay | pays | pays from the agreed degree |
| Accident in a sports club outside school | does not pay | pays | pays from the agreed degree |
| Serious illness such as cancer, diabetes, epilepsy | does not pay | does not pay | pays |
| Congenital impairment | does not pay | does not pay | usually excluded |
| Health questions when applying | none | few or none | extensive |
| Premium per year | no separate premium | about EUR 50 to 100 | about EUR 107 to 300 |
A child has no occupational risk class, and for adults that class is the strongest price driver. That is why the premium is small, and why saving on the base sum (Grundsumme) makes no sense: raising it from EUR 100,000 to EUR 300,000 costs less than most people expect. A child with a lasting impairment has a whole life ahead, and the money has to last for decades. Progression of 225 to 350 percent belongs in the contract, because costs only really rise at high degrees of disability.
The death benefit is the wrong item to buy. It covers a funeral but replaces no income, because a child has none, and policy terms cap it for minors anyway. Four other points in the terms matter far more: rescue and recovery costs (Bergungskosten) with a decent limit, cosmetic surgery and dental work after an accident (kosmetische Operationen), infections after a tick bite included explicitly (Infektionen), and the contributory effect of illness (Mitwirkung von Krankheiten) only counted from a high share, or excluded altogether. That last point decides a lot of money in a dispute.
If the budget only stretches to one policy, the money is better placed in child disability insurance than in accident insurance. Do not pick the cheaper one because it is cheaper; pick the one that carries the larger risk.
You also do not need a second accident policy if the child is already included in a family accident policy, a new contract for a child whose impairment has already been recognised, because neither policy pays for that, or a savings or education policy with a few euros of accident cover attached. If you want to build up money for your child, keep saving and insuring separate.
Questions and answers
Yes, by the statutory scheme and at no extra cost, including the journey there, school trips and class journeys. More than 1.1 million such accidents were reported in 2024.
Yes, but rarely. Out of more than 1.1 million school and day care accidents in 2024, only about 578 new accident pensions were granted.
Accident insurance asks what caused the injury and pays only after accidents. Child disability insurance asks only how severe the disability is, usually from 50 percent, and pays after illness too. It costs several times as much and asks health questions.
About EUR 50 to 100 a year for a base sum of EUR 100,000 with progression, often EUR 40 to 80 at a low cost insurer. A child has no occupational risk class, so the premium stays small.
Only if infections are explicitly included in the contract. Medically a tick bite is not an accident, but many good tariffs bring it in anyway.
Some insurers offer English service, many do not. The German wording is what counts in a claim, so ask us when a clause is unclear.
No. Neither policy is required by law, and once an impairment has been recognised, neither of them helps any more.
