Do I need insurance for my breast augmentation in Germany?
In short
You do not have to, but for a purely cosmetic breast augmentation in Germany it makes sense. If complications such as capsular contracture occur, your statutory health fund must make you share the costs under § 52 (2) SGB V; in one Federal Social Court case that was about EUR 2,300. Complication cover (Folgekostenversicherung) starts at about EUR 159 to 249 for one year and must be bought before surgery.

Worked example
Statutory health fund member, breast augmentation with implants
Two years after surgery, capsular contracture grade 4 appears and the implants have to be replaced.
Cost of the replacement operation at the fund
about EUR 4,600
Own share at 50 percent
about EUR 2,300
Complication cover, three years
about EUR 479 once
Paid by the insurance
the fund's claim
With cover you pay about EUR 479 once; without it about EUR 2,300 in the worst case, and sick pay may also be cut.
Why the German health fund does not pay everything
You pay for a cosmetic breast augmentation yourself. If the operation later causes an illness, your statutory health fund (gesetzliche Krankenkasse) treats you, but under § 52 (2) SGB V it must make you contribute a reasonable share. It must also cut or reclaim sick pay (Krankengeld) for that period, fully or partly.
The Federal Social Court (Bundessozialgericht) confirmed this in 2019. A woman's implants ruptured after a cosmetic breast augmentation and had to be replaced in an emergency. The fund asked for about EUR 2,300, and she had to pay.
What complication cover for a breast augmentation pays
Complication cover pays the health fund's claim and, for privately insured patients, the cost of further treatment. Typical cases after a breast augmentation are capsular contracture from Baker grade 3, infections and implant replacement in a second operation.
Cover only runs for a limited time, usually one to three years from the date of surgery. Capsular contracture can appear later, so providers recommend three years for breast surgery.

When you do not need insurance for your breast augmentation
If the operation is medically necessary, for example reconstruction after breast cancer, § 52 (2) SGB V does not apply and the health fund covers surgery and consequences as usual. Complication cover is then unnecessary.
If you have savings of several thousand euros for a possible implant replacement, you can also do without. Keep in mind that sick pay may be cut during a hospital stay as well.
What to check in the policy
Check that capsular contracture is named and from which grade it pays. Look at the term, the limit for private clinics and whether the clinic must be in Germany.
Many clinics sell complication cover as part of the package. That is convenient, but the scope does not always fit a breast augmentation with implants.
Step by step
Check whether the operation is medically necessary; then the fund pays as usual.
For a cosmetic procedure, choose complication cover that names capsular contracture.
Prefer three years, because breast complications can appear late.
Buy the policy at the latest 24 hours before surgery.
Keep the surgery report and your implant card safe.

Questions and answers
Frequently asked
Does the German health fund pay for capsular contracture after breast augmentation?
It treats you, but after a cosmetic operation it must make you share the cost under § 52 (2) SGB V.
Do I need the insurance if my clinic gives a guarantee?
Check whether the guarantee also covers the health fund's claim under § 52 (2) SGB V.
If it only covers a new operation at the clinic, that gap stays open.
Can I buy cover after surgery?
No.
Depending on the provider, the deadline is 24 hours before surgery or the evening before.
What about malpractice?
It is not covered.
Claims for malpractice are made against the surgeon or the clinic.
Does this apply if I am privately insured?
Whether your private health insurance pays for complications is set out in its terms.
Complication cover has its own limit for private clinics, often up to EUR 10,000.
