Who pays for weight loss injections in Germany: public insurance, private health insurance or a supplementary policy?
In short
Who pays for weight loss injections in Germany: public insurance, private health insurance or a supplementary policy? Public insurance does not pay; the law excludes drugs for regulating body weight. Private health insurance pays if prescribed, medically necessary and not excluded by the tariff. For a supplementary policy only its terms decide. Paying yourself costs about EUR 130 to almost 500 a month.

Why public insurance does not pay for weight loss injections
German social law excludes drugs mainly used to improve quality of life. It names drugs for weight loss, appetite control and regulating body weight explicitly. Weight loss injections fall under this rule, even with a doctor's prescription.
The only exception is certain diabetes treatments, where the fund pays for the drug as diabetes care, not as a weight loss injection. Weight loss pills are not paid for weight reduction either.
When private health insurance pays for weight loss injections
Private health insurers in principle treat weight loss injections as reimbursable drugs. Three conditions must all be met.
Condition
What it means
Doctor's prescription
based on the drug's approval
Medical necessity
not cosmetic, and diet and exercise have demonstrably not worked
No exclusion in the tariff
the terms do not exclude weight reduction drugs
The approval criteria matter
Weight loss injections may only be prescribed alongside a reduced calorie diet and more exercise. Adults need a BMI of at least 30, or at least 27 with a weight related condition such as high blood pressure, prediabetes or sleep apnoea.
Some drugs are approved from age 12 for children who are obese by growth chart and weigh at least 60 kilograms. Anyone wanting to lose a few kilos does not meet the criteria, and then private insurance will not pay either.
Supplementary policies and weight loss injections
For people with public insurance, an outpatient supplementary policy (ambulante Zusatzversicherung) is no reliable route to weight loss injections. Whether it pays depends only on its terms: does it reimburse prescription drugs at all, and does it exclude weight reduction drugs?
If you are already treated for obesity, you must state this when applying, as far as the insurer asks in writing. The insurer can then exclude it or decline, so a policy taken out just for planned injections rarely pays off.
Clarify the costs before the first weight loss injection
At EUR 130 to almost 500 a month, a year quickly costs more than EUR 2,000. From that expected amount, privately insured people have a legal right to a binding answer on what the insurer pays. It must reply within four weeks, in urgent cases within two.
Attach the doctor's letter with BMI, related conditions and previous weight loss attempts. The clearer the medical need, the faster the decision.
When weight loss injections are not the right choice
Weight loss injections are not for a few kilos before a holiday. Muscle mass is lost along with fat, and without exercise and a changed diet the weight returns after stopping.
Be careful with online prescriptions: consumer advice centres (Verbraucherzentralen) consider platforms without identity and weight checks unlawful. Private insurance needs a proper prescription anyway.
Step by step
Check with your doctor whether you meet the approval criteria.
Have previous diet and exercise attempts documented.
Check your tariff terms for an exclusion of weight reduction drugs.
Ask your private insurer for a binding answer, with the doctor's letter.
Start treatment only after confirmation.
Checklist
BMI 30, or 27 with a related condition
Prescription based on the approval
Proof of previous attempts
No exclusion in the tariff
Written confirmation before starting
Common mistakes
Relying on an online prescription without seeing a doctor
Starting without confirmation and being left with the bill
Taking out a supplementary policy just for the injection
Believing the public fund pays with a medical certificate
