Does supplementary dental insurance pay for periodontitis in Germany?

In short

Does supplementary dental insurance pay for periodontitis in Germany? Only for what public insurance leaves open, and only if the disease was unknown when you signed. Public insurance pays the systematic treatment after approval, plus two years of aftercare. You pay for cleaning at EUR 80 to 150, laser or local antibiotics, often EUR 30 to 150 per tooth.

Worked example

Periodontitis three years after signing

A man aged 52 has had a plan with treatment and prevention for three years. His dentist diagnoses periodontitis and recommends extras on six teeth.

Basic therapy and aftercarepublic insurance
Cleaning beforehandEUR 120
Laser on six teethEUR 540
Reimbursed at 90 percentEUR 594

He pays EUR 66 instead of EUR 660. Had the periodontitis been known when he signed, he would pay the full EUR 660.

What public insurance pays for periodontitis

Periodontitis is the bacterial inflammation of the gums and jaw bone around the teeth, often called Parodontose in everyday German. Since July 2021 the PAR guideline (PAR-Richtlinie) sets out what statutory health insurance pays. Your dentist applies before treatment and the insurer decides.

Covered are deep cleaning of the gum pockets, open surgery where needed and supportive aftercare for two years, extendable by up to six months on request. You are expected to keep up good oral hygiene.

What stays private

Many practices recommend a professional cleaning first, which you pay yourself. Extras such as laser, antibiotics placed into the pocket or rebuilding bone with membranes are also private.

Supplementary dental insurance pays exactly these items for periodontitis if it includes dental treatment and prevention. Plans covering only Zahnersatz pay nothing here.

The hurdle when you apply

Nearly every application asks about gum disease and ongoing treatment. If periodontitis is known or has been treated, many insurers exclude it or decline. Some plans skip health questions but then only pay for new, future treatment.

Answer accurately. Hiding periodontitis can cost you the whole contract, not just one payout.

When a policy for periodontitis is not worth it

Once diagnosed, no new contract pays for the current treatment. The public benefit remains, and the extras of a few hundred euros are on you.

A plan with prevention makes sense before problems start: it pays yearly cleaning and covers extras later if periodontitis appears after you signed.

Step by step

  1. Wait for the diagnosis and the application to your health insurer.
  2. Ask for a written cost plan for extras.
  3. Check whether your plan covers treatment and prevention.
  4. Get written confirmation from your insurer before starting.
  5. Keep your aftercare appointments to avoid a relapse.

Checklist

  • Periodontitis unknown when you signed
  • Plan covers treatment, not only Zahnersatz
  • Prevention budget included
  • Public insurance application approved
  • Payout caps for the first years checked

Common mistakes

  • Signing up after the diagnosis and hoping for payment
  • Answering gum health questions vaguely
  • Dropping aftercare and putting the result at risk
