Who pays when the drain is blocked in Germany? No insurance pays for the blockage itself, because the pipe is not broken. In your own house you pay for drain cleaning, often around EUR 280 a call out. In a rented flat the landlord pays unless you caused the blockage. If heavy rain pushes water back from the sewer, only natural hazards cover helps.

Worked example
Kitchen drain blocked in a detached house, water runs into the base cabinet
How the costs split when only the blockage is cleared and minor damage repaired.
Drain cleaning with machine
around EUR 280, paid by you
Swollen base cabinet of the fitted kitchen
about EUR 400
Floor under the kitchen damp, short drying
about EUR 400 to 800
Buildings insurance excess
for example EUR 250
Drain cleaning always stays with you. Whether buildings or contents insurance covers the damage to kitchen and floor depends on the terms, and for small amounts the excess eats up much of it.
Why insurance does not pay for clearing a blocked drain
Buildings insurance pays for pipe water (Leitungswasser) when water escapes where it should not or a pipe breaks. A blocked drain is neither: the pipe is intact, the water just cannot flow away. Clearing the blocked drain is therefore maintenance, not damage.
In your own house you pay for clearing the blocked drain yourself. The model terms even require you to keep the drain pipes on the property clear. If you ignore a known blockage and damage follows, you risk a cut.
When a blocked drain causes water damage
If water overflows from a basin or shower because of a blocked drain and soaks floor and ceiling, the question is whether it counts as pipe water damage. Whether water overflowing from a connected fixture counts as escaping water depends on your terms and the circumstances. So always report such damage, with photos and the drain cleaner's invoice.

Rented flat: landlord or tenant pays for the blocked drain
The landlord must keep the flat in a condition fit for use during the tenancy (section 535 Civil Code). A working drain is part of that. Tenants are not responsible for normal deposits from normal use (section 538 Civil Code), so the landlord then pays for drain cleaning.
It is different if the tenant blocked the drain with food scraps, cat litter or wet wipes. Then the landlord can recover the cost. As a tenant, report a blocked drain early and only call the drain cleaner yourself in a real emergency.
Backflow from the sewer after heavy rain
Backflow (Rückstau) in insurance terms means heavy rain or flooding pushes water from the public sewer back through the drain pipes into the house. That is not a blocked drain but natural hazard damage (Elementarschaden). It is only covered if your policy includes the natural hazards module.
If you have rooms below street level with a floor drain, you need backflow protection that is kept in working order. If it is missing or faulty, the insurer may cut the payment after a backflow for the breached duty.
Preventing a blocked drain
Most blocked drains start in the kitchen and bathroom: fat, coffee grounds, hair and wet wipes. A strainer in the drain and hot water after washing up help more than harsh cleaners that attack old pipes.
If the drain keeps blocking in the same place, have the pipe inspected with a camera. Root ingress or a sagging house connection can be found before a pipe breaks.
Step by step
Stop using the water and mop up overflowing water immediately.
Photograph the damage and the blocked spot.
As a tenant inform the landlord, as an owner call a drain cleaner.
Ask for an invoice stating the cause, such as fat or roots.
Report damage to floor or ceiling to the insurer.
If the blockage keeps coming back, order a camera inspection.
