Does accident insurance in Germany pay for cosmetic surgery after an accident?
In short
Yes, if your accident insurance in Germany includes the cosmetic surgery add-on (kosmetische Operationen). After an accident it pays for surgery that repairs a lasting scar or disfigurement, often dental implants for natural teeth too, up to a fixed amount. If health insurance or the other party's liability insurer pays, you only get the rest. Without the add-on, the surgery itself is not paid.
Published costs for typical treatment after an accident and for cover in Germany.

Typical range
Single implant, front tooth
about EUR 1,500 to 3,000
Single implant, back tooth
about EUR 1,200 to 2,500
Private accident insurance, adult office worker, base sum EUR 100,000, progression 350 percent
about EUR 60 to 180 a year

Your premium depends on age, job and add-ons; we obtain the exact quote for you.
Worked example
Bike fall, age 34, scar on the forehead and a broken front tooth
After healing, a wide scar remains above the eyebrow and one front tooth is lost. Health insurance pays a subsidy towards the tooth but refuses the scar revision as not necessary.
Scar revision, outpatient
about EUR 2,000 to 4,000 (guide value from quoting practice)
Implant with crown for the front tooth
about EUR 1,500 to 3,000
Less: health insurance subsidy for the tooth
as stated in the insurer's decision
Cosmetic surgery add-on
EUR 10,000
Total cost about EUR 3,500 to 7,000. With the add-on, accident insurance refunds whatever health insurance does not pay. Without it, you pay that remainder yourself.
What the cosmetic surgery add-on pays
The core benefit of private accident insurance (private Unfallversicherung) is the disability payment based on the body-part schedule (Gliedertaxe). It reflects loss of function, for example of an arm or an eye. A facial scar without loss of function usually triggers no disability payment. That gap is what the separate cosmetic surgery add-on covers.
Typical terms refund proven, necessary costs for doctors' fees, the operation, medication, dressings, hospital accommodation and meals, plus dental treatment, dentures and laboratory costs, up to the agreed amount.
Situation after an accident
Does the add-on pay?
Visible facial scar is corrected
yes, after treatment is complete
Broken natural front tooth, implant
yes, if dentures are included
Damaged existing bridge, crown or implant
explicitly no in many terms
Scar treatment paid by health insurance
only the part health insurance does not pay
Cosmetic surgery unrelated to an accident
no
Surgery after the deadline
no
Deadlines and conditions
A doctor must carry out the operation once medical treatment has ended. One published set of terms requires adults to have the operation within five years of the accident, and children before their 21st birthday. Other policies have shorter deadlines, so check yours before planning surgery.
For teeth, such terms treat all natural teeth except milk teeth as part of your appearance. They exclude costs for existing artificial dentures such as bridges, crowns and implants, and jaw treatment.
Who pays first
The add-on is secondary. It pays where no third party, such as your health insurer or the liability insurer of the person who caused the accident, has to refund the costs, or where that third party disputes its obligation. Submit invoices there first.
Statutory health insurance pays for treatment that is necessary under Section 27 of Social Code Book V (SGB V) to cure an illness, prevent it from getting worse or relieve symptoms. A scar that only affects your looks often does not qualify. After an accident at work or on the way to work, statutory accident insurance is responsible, which under Section 26 SGB VII must use all suitable means to remedy damage to health and reduce its consequences.
When you need the add-on
For children, people who play sport and anyone who cycles or spends time on the water, the add-on belongs in the policy. Falls often affect the face and teeth, and a single implant can easily cost several thousand euros.
If you already have private health insurance with generous dental refunds, a smaller amount may do. Leaving it out entirely rarely makes sense, as the extra premium is usually small.
Step by step
Have scars and teeth photographed and documented by a doctor after the accident.
Report the accident to your accident insurer within the deadline.
Once treatment is complete, get a cost estimate for the operation.
Submit the estimate to your health insurer or the other party's liability insurer first.
Send the refusal or the remaining amount with the invoices to your accident insurer.
Keep the policy deadline in mind and plan the operation in time.
