Which insurance do employed doctors in Germany need?
In short
Which insurance do employed doctors in Germany need? Your own professional liability as a top-up to the hospital policy, because the professional code requires adequate cover and side jobs are usually not included. Plus occupational disability insurance, because the medical pension fund (Versorgungswerk) only pays once you give up medicine almost completely. At 27, a EUR 1,500 pension starts at about EUR 48 a month.
Senior physician, municipal hospital, 50, EUR 3,000 disability pension
from around EUR 240 to 410 a month
Professional liability for doctors, all forms
about EUR 225 to 6,500 a year, from EUR 18.75 a month
Personal liability, single
about EUR 40 to 120 a year
As of October 2026, from publicly quoted examples and legal texts.
That top-up policies for employees sit at the low end of the range,
and the amounts in the worked example, are guide values from our quoting practice.
Your premium depends on specialty, age and health; we obtain the exact quote for you.
Worked example
Resident, 29, internal medicine, with freelance emergency doctor shifts
The hospital covers ward duty, but not the emergency shifts on her own account.
Occupational disability, EUR 2,000 pension to 67
about EUR 65 to 110 a month
Own professional liability including emergency work
about EUR 30 to 80 a month
Personal liability
about EUR 4 to 10 a month
Together about EUR 99 to 200 a month. Without freelance shifts, a cheap top-up to the hospital policy is often enough.
What the hospital and pension fund already cover for employed doctors
For mistakes on duty, the hospital operator normally answers to patients, and employed doctors are covered by its business liability policy. The model professional code (Musterberufsordnung) still obliges every doctor to have adequate liability cover for professional work, explicitly including employees. Side jobs, expert reports and help off duty are usually not covered by the hospital.
As members of a medical chamber and its pension fund, employed doctors can be exempted from the statutory pension scheme. The pension fund then pays on disability, but only once you give up medical work almost entirely. Some funds even require you to return your licence (Approbation). You still have statutory accident insurance as an employee, for accidents at work, on the commute and infections on duty.
Priorities: what employed doctors should insure first
Priorities follow the gaps: liability outside the hospital policy and losing your ability to work in your specialty weigh most.

Disability cover: specialty and infection clause
Premiums depend heavily on whether you operate. A 27-year-old internal medicine resident without surgery pays from about EUR 48 for a EUR 1,500 pension, a 38-year-old surgeon from about EUR 140 for EUR 2,500. Starting early costs less, because age and medical findings drive the price.
An infection clause matters for doctors. If the health authority bans you from patient work under the Infection Protection Act, for instance after a hepatitis or HIV infection, the policy with this clause pays even if you feel well. Without it, insurers are often not liable in such cases.
Liability beyond the hospital: when employed doctors need their own policy
Employed doctors mainly need their own professional liability for work outside the employment contract: emergency or locum shifts on your own account, expert reports, vaccination drives, volunteer work and helping in an emergency on a train. Add employer recourse for gross negligence.
Ask the hospital to confirm in writing who is covered by its policy and for how much. Hospitals do not always have a policy that includes junior doctors and doctors in specialist training.
Health insurance: statutory or private
If your salary is above the compulsory insurance threshold of EUR 77,400 a year in 2026, you can choose between statutory and private health insurance. For doctors, family plans, pre-existing conditions and adequate daily sickness benefit (Krankentagegeld) for long absences matter more than today's premium.
What employed doctors usually do not need
Practice contents or business interruption cover is only for your own practice; there are separate pages for business types. Accident insurance does not replace disability cover. And a liability policy with full practice cover is too much if you only work as an employee with the odd side job.
Step by step
Get written confirmation from the hospital whether and how much you are covered.
List all work outside your contract, including occasional jobs.
Apply for disability cover with infection clause, ideally while studying or early in training.
Read your pension fund's rules on disability.
If you earn over EUR 77,400, choose health insurance with family and sick pay in mind.
